6/9/2023 EnchantaVaughn Approved 1.Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a jovial mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Kaila expressed to QMHPA that she was having a blessed day. QMHPA informed Kaila that the purpose of this session was going to focus on decreasing and coping with feelings of depression to assist with stabilizing her mood. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: During this second portion of the session, Kaila was observed participating in the session with what appeared to be a confused look when discussing with her ways on how to manage her anger. 3.Behavior: During the third portion of the session, Kaila was engaged in the session and energized when learning new ways to handle her anger and know she is in control over her own feelings. 1.Intervention: QMHPA engaged Kaila in a discussion surrounding her mental health. QMHPA openly discussed problems areas that contribute to her daily well-being. QMHPA explored problem-solving techniques to utilize when feeling overwhelmed and having anxieties. QMHPA encouraged positive self-talk for the Kaila to reframe from negative thinking and acting out negatively. QMHPA engaged Kaila in a skill building activity. QMHPA explained to Kaila the purpose of this activity was to assist Kaila with reducing stress evident by deep breathing to calm herself, and encouraged thought-stopping techniques to replace negative thoughts before acting out with anger and frustrations. 2.Intervention: QMHPA provided Kaila with praise for expressing and acknowledging that she needs support. QMHPA engaged Kaila in skills building activity. QMHPA informed Kaila that the purpose of this activity was to assist her in identifying anger management skills that would support her with reducing her aggression and redirecting her anger into more positive activities. QMHPA explained to Kaila that another purpose of this activity was to assist her with relaxation skills to help Kaila with calming herself when feeling agitated and frustrated by others. QMHPA processed Kaila feelings and thoughts after the skills building activity. QMHPA ended the session by scheduling the next sessions with her for the upcoming week. 3.Intervention: QMHPA addressed Kaila signs of depression and her ability to improve depressive symptoms (frumpy appearance and defeated feelings associated with failed relationships). QMHPA utilized active and attentive listening skills as Kaila discussed her frustrations with QMHPA about her relationship with her family. QMHPA assisted Kaila with learning to recognize and verbalize hurt and angry feelings in open assertive ways. QMHPA thoroughly assessed the various stimuli that have triggered Kaila anger and the thoughts, feelings, and actions that have characterized her anger responses. QMHPA conducted an directive individual anger management therapy session and assisted Kaila in identifying ways that key life stressors have increased her anger and how these stressors have positively or negatively influenced the way she handles anger. QMHPA continued to implement anger management techniques to further assist Kaila with maintaining a stabilized mood so that she can decrease the moments in which she is frustrated and/or depressed. 1.Response: Kaila appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Kaila shared with QMHPA that her mental health was fine; however, she has been feeling a little depressed. Kaila expressed to QMHPA that she doesn’t know where the depression is coming from. During the skills building activity, Kaila was understanding of how the skills would assist her with calming herself, and using relaxation techniques to anger negative decisions. Kaila expressed to QMHPA that she thinks that she needs to get an updated psychological; however, she knows there is a waiting period due to QMHPA assisting with scheduling one weeks’ ago. 2.Response: QMHPA reiterated that it would better assist her with identifying any concerns that she may have about herself. Towards the end of the session, Kaila expressed to QMHPA that she as going to try to use those skills that were taught to her during the session. She indicated to QMHPA that she was a little depressed and angry due to one of the workers at her job just up and quitting and without communicating with her. During the skills building activity, Kaila reported that she doesn't understand were her anger stems from anymore as she reports she feels as though everything just irritates her at this point because of everything she got going on with her personal life. 3.Response: Kaila stated that she continues to struggle with applying techniques when faced with frustrating situations. Kaila actively participated in activity as she was able to openly discuss stressors in her daily living and continued to stress the fact of needing to move to improve her life. Kaila stated that she was willing to make changes within her life but she need to be in a home by herself. Kaila informed QMHPA that her only concern was assuring that she is able to abide by the budgeting strategies to maintain her daily living. Kaila demonstrated a positive attitude while in the community as she stated that it was a breather and she needed to be out of the house. Kaila stated that it was easier for her to say that she was going to maintain a stabilized mood than to actually do it, however she stated that she would continue to work harder at utilizing techniques.
